Transfer Agreement for University Archives and Records Program

This form is used when a new record group is transferred to the archives, or when a significant addition is made to an existing record group. The completed form remains on file.

Transfer Agreement for Records Center Storage

This form applies to exceptional cases involving records typically lacking permanent historical value which must be retained for legal or fiscal reasons. For information on the applicability of Record Center Storage, contact Nancy Bartlett (nbart@umich.edu).

Transfer Agreement for Faculty and Individuals

This form is used for faculty and other individuals transferring records to the archives. See accompanying Gift Agreement document for further information about the form.

Digital Files Transfer Form

This form should accompany all transfers of digital files or files on digital media, including diskettes, CD-ROMS, and DVDs. For more information on transferring digital files, please consult the transfer guidelines included in section 5.1 of the Records Policy and Procedures Manual.

Loan Form

This transactional form is used to track and administer original records loaned back to the creating unit. The form is initiated by the archives staff.

Transfer Agreement for Student Papers

In order for papers written by students to be available for research, this completed form must accompany any student authored papers approved for deposit in the archives.

Oral History Agreement

This form, which grants researchers access and releases copyright to the material, must accompany transfers of oral history tapes and transcripts.

Patient/Client Record Access Agreement

This form, designed to protect confidentiality, is required in order to conduct research in records known to contain personally identifiable patient/client information.
